Music Together in the Tri-Valley

Re-enrollee form: for families that have taken a Music Together semester before.

By registering online you agree the accept the Music Together in the Tri-Valley regulations and conditions.

1st & 2nd Class choices (e.g. 1st: Friday , Livermore 10:30am, 2nd Thursday Pleasanton 9:15)

This will take you to the Paypal website. To add a second child press the "continue shopping" button at Paypal and fill in second child details below

2nd Child (sibling) registration (8 months and over as of 03/30/10) - $125

Note: if you would like to register three children, please use this registration form instead to take advantage of our special three-sibling rate.

Once the Paypal payment has been made, you will receive an email from Paypal to confirm that payment has been sent and, within 2/3 days, another email from Music Together to confirm your place in the class.

Note : If you have a 2nd Child (sibling) who is under 8 months as of 03/30/10 they are FREE so please simply email or phone us (925 551 7722) with the child's name and DOB.

No refunds will be given after the second class.
The new family $15.00 registration fee is also non-refundable.
An administrative processing fee of 10% of the tuition payment is non-refundable.
Materials: 2CD pack, songbook and guidebook are non-returnable unless undamaged, still sealed & suitable for resale.
Registrations are non-transferable. If you are unable to take a class for which you have paid, please call 925-551-7722.
Up to 3 make-up classes per semester are permitted. Missed classes will not be credited back or carried on to next semester. Schedule your makeups at makeups.musictogether.net

I agree to waive Music Together in the Tri-Valley, Inc., and its staff and teachers, Music Together LLC, The city of San Ramon, Triple Threat Performing Arts, Tao Sports, Contra Costa Ballet Centre, Ingram and braun Musik shoppe, The city of Walnut creek and its staff, Blackhawk Plaza and centerCal corp, and their owners of all claim for personal injury or property damage that may arise from use of the facilities before, during, or after Music Together classes. I also accept responsibility for my safety and the safety of my children at the studio before, during, and after Music Together classes.
